At Pioneer Health we now offer the service of a Credentialed Diabetes Educator. A Credentialed Diabetes Educator is a health professional who has completed further study focused specifically on helping people with diabetes self-manage their condition to prevent or prolong the onset of complications. Credentialed Diabetes Educators can offer you individualised information, support and motivation to assist you in managing your diabetes to the best of your ability. They specialist in troubleshooting any issues that may be affecting your self-management. Credentialed Diabetes Educators can also be the link between you and other allied health services that may assist you in your journey.
It is recommended by Diabetes WA that all people with diabetes and those who are pre-diabeteic, see a Diabetes Educator at least annually as part of their annual cycle of care. Our Credentialed Diabetes Educator can be seen by arranging to have a GP Management Plan completed with your usual GP. Once the GP Management plan is done access to our credentialed Diabetes Educator is bulk billed through eligibility for your Allied Health Professional visits.
